Hi

I added a document and document's form to extension. Then I created some attributes in extension but they are not listing in object list. How can I use attributes which are I created.

indeed, in this case the added attribute will not be available (screenshot 1)

You could do the following: add a form object to the extension (screenshot 2). After that, all new attributes will be available (screenshot 3).
